JOB SUMMARY AND PURPOSE

The Case Manager provides intensive case management services to gang-involved youth and families impacted by gang violence. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. Each Case Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available as described below.

DUTIES AND RESPONSIBILITIES

  • ASSESSMENT/CASE PLANNING: Conducts comprehensive screenings and assessments with individuals and families to collect functional environmental psycho-social financial employment housing educational and health information as appropriate to develop a case plan.
    • Serves and engages gang-involved youth or young adults ages of 14 to 25 years in a GRYD Zone
    • Maintain a minimum of 30 FCM clients updated and active
    • Maintain a minimum of 10 TCS client updated and active
    • Make home visits to assist clients and family with services needed
    • Follow all phases of case management
    • Run weekly reports to maintain data base error free and up to date
  • Meet weekly with all Community Intervention Workers
  • Audit TCS files of Community Intervention Workers weekly
  • Report weekly on progress of case management
  • Apply Social Embeddedness Tool (SET) to youth as required.
  • Participate in planning meetings and work at Summer Night Lights (SNL) and Fall Friday Nights (FFN) sites
  • Strategically plan and work with community to decrease and eliminate gang activity and violence
    • Plans strategies to engage local gangs/crews proactively maintain peace via safe passages and activities
    • Work with Law enforcement to create peace in the community
    • Maintain weekly contact with schools community based organizations and gangs in the area
    • Work with multiple gangs in the area
    • Facilitate workshops Ex. life skills substance abuse etc.
    • Do street outreach and pass out literature and services
    • Create monthly calendar of activities
    • Create monthly community engagement parent and client activities
    • Travel and drive for program outings and activities
    • Transport clients as needed
    • Keep track of community service workers
  • Accompany youth to probation appointments and submit monthly probation progress reports.

ESSENTIAL DUTIES:

  • Assessment: through individual meetings on a regular basis and as part of Case Management team develop a detailed plan of action to meet the clients needs set goals and find the necessary resources to meet those goals
  • Strategic efforts at Gang reduction in community plan and execute gang reduction events and activities.
  • Meet with and Audit files/efforts of Community Intervention for participants before other agencies/community settings
  • Other duties as required and is subject to change at any time.

Qualifications

REQUIREMENTS:

  • Must be able to pass a fingerprint clearance background check including criminal history personal references employment and education verifications

EXPERIENCE:

  • 2 years experience working with At-risk families/individuals or gang population

PREFERRED QUALIFICATIONS:

  • LAVITA (Los Angeles Violence Intervention Training Academy) certification
  • Trained on the GRYD Model of practice and the Efforts to Outcome (ETO) Database
  • Bilingual (Spanish preferred)
