Job Summary

Prepare and dispense medications accurately and efficiently for long-term care facilities assisted living communities and other institutional clients.
Conduct thorough medication reviews to ensure appropriateness effectiveness and safety of prescribed therapies.
Collaborate with healthcare providers and facility staff to optimize medication therapy and enhance patient outcomes.
Provide medication counseling and education to facility staff and caregivers on proper medication administration and potential side effects.
Monitor patient outcomes and medication adherence and adjust therapy as needed.
Ensure compliance with all federal state and local regulations related to pharmacy practice and medication dispensing.
Manage inventory and procurement of medications and supplies ensuring proper storage and handling.
Participate in quality assurance programs and continuous improvement initiatives to enhance pharmacy services.
Stay current with best practices and advancements in pharmaceutical care for institutional settings.
Maintain accurate and up-to-date patient records and documentation.

Preferred Qualifications:

Bachelors of Pharmacy or PharmD degree from an accredited pharmacy program.
Experience in closed-door or institutional pharmacy.
State pharmacy license in good standing.
Proficiency in preparing and dispensing medications for institutional clients.
Strong analytical and problem-solving skills.
Excellent communication and collaboration abilities.
Familiarity with electronic health record (EHR) systems and pharmacy management software.
