St. Lukes Health - Memorial Hospital in Livingston TX is located just off Highway 59 south. The 66-bed hospital provides more than 60000 patient services each year with half of those services being offered in the Level 4 Trauma-Designated Emergency Department. Services offered include Orthopedic care Womens Services and a state-of-the-art Sleep Center. Imaging services include MRI CT scan nuclear medicine scans 4D Ultrasound cardiac echocardiograms fluoroscopy bone density scanning and 3D mammography.
Job Summary and Responsibilities
As our Clinical Pharmacist youll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury. Youll review prescriptions for potential drug interactions correct dosing patient allergies or anything that looks unusual in your opinion.
Essential Key Job Responsibilities:
- Demonstrates ability to document clinical activities and interventions such as consults patient teachings and other patient specific information as defined by policy and procedure.
- Interprets medication orders and prescriptions for drug interactions appropriate drug selection and appropriateness.
- Accurately enters the order or prescription into the computer system labels and dispenses medications in a timely manner.
- Resolves drug related problems by consulting with other health care professionals to achieve appropriate patient outcomes.
- Assists the licensed provider in appropriate drug selection and therapeutic regimens.
- Provides patient counseling and drug information and documentation as appropriate.
Job Requirements
- Bachelor of Science in Pharmacy (B.S.) OR Doctor of Pharmacy (Pharm D) degree from a Board approved ACPE Accredited College (pdf) of pharmacy program OR
- Graduated from a college of pharmacy program outside the U.S. and have obtained full certification from the Foreign Pharmacy Graduate Equivalency Committee (FPGEC)
- Licensed Pharmacist in the State of Texas through Texas State Board of Pharmacy (TSBP)
- Basic Life Support (BLS) certification (through an approved AHA program)
Preferred
- Two (2) Years of experience as a Clinical Pharmacist in an acute care setting
- Masters degree in business administration or similar field of study
- Completion of pharmacy administration residency
- Completion of pharmacy (PGY-1) residency
