Licensed Practical Nurse (LPN) Wilson Rehab and Nursing Care (WRNC)
$15000 Commitment- Incentive Bonus
Schedule: 5x8 (3p-11p) rotating weekends/rotating holidays

More about our team
Join our team in a 90-bed Skilled Nursing & Rehabilitation Unit with 20 Rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs) Licensed Practical Nurses (LPNS) Certified Nursing Assistants (CNAs) and Medication Aides. Operating with a patient ratio of 1:15 days 1:19 evenings and 1:25 nights our unit cares for patients with Congestive Heart Failure (CHF) COPD Diabetes ESRD Renal Failure Dementia Stroke and heart disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.
How youll contribute
A Licensed Practical Nurse (LPN) who excels in this role:
Accurately performs patient assessments and identifies patient needs
Identifies and initiates appropriate nursing interventions
Provides care appropriate to condition and age of the patient
Performs timely and appropriate documentation relating to medical necessity in the medical record
Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
Performs timely and accurate QI assessments

What were looking for- a compassionate nurse with a strong foundation in providing holistic and compassionate long-term care with a focus on detailed patient assessment medication administration and chronic disease management.
Applicants should have a current state LPN license and possess a degree from an accredited nursing school. Additional requirements include:
Basic Life Support certification is required within 30 days of hire.
Long-term care (LTC) preferred or desired to work with LTC patients
