Contract Duration: 4 8 weeks (potential extension)
Schedule: 5 days/week possible weekend rotation

Position Overview:
We are seeking a compassionate Licensed Practical Nurse (LPN) to provide exceptional care in a long-term care/skilled nursing facility. Ideal candidates will have a minimum of one year experience in LTC/SNF settings be detail-oriented and able to work in a fast-paced collaborative environment.

Responsibilities:

Provide direct patient care according to individualized care plans
Administer medications and treatments as prescribed
Monitor and document patient condition changes
Support admissions transfers and discharges
Maintain accurate documentation in EMR
Collaborate with interdisciplinary team
Ensure a safe clean environment

Requirements:

Active MA LPN license
Current CPR/BLS certification
Minimum 1 year recent LTC/SNF experience
Flexible availability including weekends
Strong clinical and communication skills
Detail-oriented with commitment to quality care
