Responsibilities:
- Conduct physical exams and patient assessments.
- Diagnose and treat common acute illnesses.
- Manage chronic health conditions such as diabetes hypertension and asthma.
- Prescribe medications and monitor patient responses.
- Educate patients on health maintenance and disease prevention.
- Collaborate with healthcare team members to optimize patient care.
- Maintain accurate and up-to-date patient medical records.
- Participate in quality improvement initiatives to enhance healthcare delivery.
Requirements:
- Masters degree in Nursing with specialization in Family Medicine or Internal Medicine.
- Active Nurse Practitioner license in the state of practice.
- Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
- Strong clinical skills and ability to work independently.
- Excellent communication and interpersonal abilities.
- Empathy and compassion towards patients of diverse backgrounds.
- Proficiency in electronic health records (EHR) systems.
- Commitment to ongoing professional development and learning.
Schedule: Monday-Friday 9a-5p
No call shifts required
Patient Census: 50 per week
Patient Population: Ages 15 or 18
Must be board-certified or board-eligible
