Community Action Partnership has an exciting opportunity for

Community Health Worker

In this role the Community Health Worker (CHW) will assist the Family Resource Centers and/or CalAIM Housing Navigation and Enhanced Care Management programs by providing support to participants. The CHW will act as an intermediary between their participants and healthcare and/or social services providers playing a critical part in the work and dedication of Community Action Partnership of Orange Countys initiatives. Its leadership with a cause and the rewards are immeasurable!

WHAT YOU WILL ACCOMPLISH IN THIS ROLE

The Community Health Worker will function as an integrated team member by providing support for the FRCs and/or Enhanced Care Management and Housing Navigation programs. The Community Health Worker will provide support to the most vulnerable population including but not limited to individuals experiencing homelessness high utilizers and or youth involved in child welfare by helping them navigate their health and social needs. Duties performed in this position will require bilingual English/Spanish written and oral skills.

  • The salary for this position is $23.00

The work schedule for the Community Health Worker does not allow for a remote or hybrid work evening and weekend hours may be required.

ROLES AND RESPONSIBILITIES

  • Engage with the community through various outreach activities and distributions through a screening process to help clients access and connect to case management services.
  • Collaborate with partners providing programs and services for participants of the FRCs and CAP OC.
  • Assist with FRC programs and services including but not limited to front desk and in office coverage food and diaper distributions seasonal activities senior programs workshops health education etc.
  • Play an active role within the ECM and Housing Navigation program by assisting members in completing forms applications and or intakes required to receive needed services or community resources. Provide client support in the field as needed. Including clients home shelter hospital and skill nursing facilities etc.
  • Provide tailored support to help members achieve their goals.
  • Provide and facilitate members with community resources referrals or services as needed.
  • Gain familiarity with eligibility requirements for the Medi-Cal and CalFresh programs
  • Assist participants through application process for Medi-Cal and CalFresh
  • Conduct outreach through home visits.
  • Gather clinical information and assist with coordinating post-discharge services including scheduling provider appointments ensuring post-discharge referrals are received by the member transportation to appointments is arranged and members are aware of follow-care needs
  • Assist members with the coordination and navigation of the health care system and processes.
  • Provide culturally appropriate health education and information to members.
  • Educate members about preventative health measures and healthy lifestyle choices.
  • Assist with conducting member needs and or health assessments.
  • Assist members with move-in tasks such as but not limited to identifying and coordinating the purchase and delivery of housing deposit items.
  • Accompany members to office visits as needed.
  • Visit and monitor members at the hospital/skill nursing facility etc.
  • Participates in agency team meetings and training as required.
  • Complete and submit program reports on a scheduled basis or as needed.
  • Maintain accurate records of all patient contacts
  • Participates in community events health fairs and or supportive services events
  • Employ motivational interviewing trauma-informed care and harm-reduction approaches.
  • Fulfill other duties as assigned.

THE IDEAL CANDIDATE HAS KNOWLEDGE AND EXPERIENCE IN

  • Knowledge of homeless and housing services case management financial literacy financial stability and poverty issues.
  • Knowledge of community resources and experience working with a variety of community-based organizations preferred.
  • Planning and organizational techniques to develop and implement action plans.
  • Basic teaching training skills and techniques and public speaking skills.
  • Analyzing and interpreting various materials to make recommendations understand and apply program rules regulations and procedures.
  • General knowledge of conducting assessments to identify members medical social and psychological needs and using various assessment tools and methodologies.
  • Excellent verbal and written communication skills.
  • Strong organizational abilities to manage multiple tasks and assignments.
  • Efficient in prioritizing tasks and ensuring timely service delivery.
  • Capable of identifying issues developing solutions and implementing changes to improve member care.
  • Strong critical thinking skills for quick informed decision-making.
  • Empathetic and skilled in building trusting relationships with members and families.
  • Understanding of member privacy laws such as HIPAA to ensure confidentiality.
  • Ability to work both independently and collaboratively within a team

  • Flexible: Work locations may vary at CAP OCs other locations such as FRCs Orangewood office etc. and working in the community which can include working outdoors in varying temperatures and weather conditions.
  • May require working weekends/ evenings/ holidays when/if needed to meet client/production demands. Workdays and hours of work are subject to change.
  • Language skilled: Must demonstrate clear concise and effective communication skills both orally and in writing in English.
  • Effectively create/present information and respond to questions from staff clients media groups and the public
  • Mathematical: Promptly and correctly create compose and complete mathematical equations on a computer and/or to complete forms for reports and/or presentations. Compile numbers statistical data and obtain other information for forms reports and presentations.
  • Computer literate: Competently use the Internet Web based databases Microsoft Office (Word Excel PowerPoint Outlook Publisher and Access) and other applications. Experience with virtual communication platforms. Use a variety of computer databases to ensure that client records statistics and reports are completed.

Qualifications

EDUCATION AND EXPERIENCE

  • Bachelors degree in health care services social work human services preferred or 2 years of college course work and 2 or more years of equivalent work experiences.
  • High School diploma or GED required.
  • Bilingual in Spanish required.
  • Current Community Health Worker certification preferred.
  • Two years of health/clinical experience or equivalent health-related education preferred.
  • Knowledge of community resources and experience working with a variety of community-based organizations preferred.
  • Experience working with low-income and at-risk populations including but not limited to individuals experiencing homelessness poverty domestic violence chronic health and mental health conditions.

TRAVEL

Possess a valid California Drivers License with a driving record that meets minimum standards established by CAP OC insurance carrier proof of vehicle insurance access to a vehicle and willingness to drive/travel when required.

Typically travel is local during the business day to/from CAP OC locations and to/from community events which are usually within Orange County; possible day travel to/from LA County and Inland Empire counties is possible. The incumbent will use their personal vehicle. Travel may be during evening hours with possible out-of-the-area and overnight travel.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job the employee is regularly required to bend stoop reach pull push stand kneel sit twist turn walk bend at the waist talk and hear prolonged and fine dexterity of fingers and wrists with prolong computer work vision abilities include close vision distance vision color vision ability to adjust focus and prolong periods of looking at a computer screen. The employee must regularly lift and/or move objects up to twenty (20) pounds. Employee will experience prolonged periods of sitting at a desk and/or standing. Employee may be called upon to work outdoors in varying temperatures and weather conditions.

A medical examination is required of each new employee whose physical condition must meet the minimum requirements prescribed for the addition prospective employees must pass a pre-employment physical drug screen Live Scan and background check.
