Job Description

Community Resource Network of Florida supports Individuals with Intellectual/Developmental Disabilities to live their best lives. Our person-centered focus allows us to assist Individuals to reach their goals and be as independent as possible.

We are seeking a full-time dependable compassionate creative and fun person to join our amazing team. Our Fort Myers Life Skills program is open Monday-Friday.

Hours are 8:00 a.m.-4:00 p.m. M-F

ROLE OF THE LIFE SKILLS ASSISTANT PROGRAM MANAGER

The Life Skills Assistant Program Manager is responsible for supporting the instructors who are working directly with Individuals with intellectual and developmental disabilities (I/DD) to achieve their highest level of independence. The Life Skills Assistant Program Manager focuses on the assurance that care is being provided using a person-centered approach. The Assistant Program Manager will provide support training and guidance in accordance with the scope of services to ensure the best outcomes towards Individuals goals.

The LSP Assistant Manager will work in conjunction with the Program Manager to build a cohesive team. The LSP Assistant Manager strives to inspire others stimulate enthusiasm and leads by example. The LSP Assistant Manager is committed to making a positive impact on others through effective relationship management.

ESSENTIAL FUNCTIONS

  1. Operations
  • Monitor the health and safety of all individuals.
  • Ensure care requirements are in compliance with the Individuals Support Plan.
  • Along with the Individual assist in the creation of each Implementation Plan Goal.
  • Implement Individuals IP goals along with the instructors.
  • Assist and guide instructors on methods to help Individuals attain their IP goals.
  • Monitor Individuals progress toward their Implementation Plan goals and track toward graduation.
  • Create and submit quarterly summary documents to WSC in accordance with Qlarant requirements.
  • Administer medication as needed if instructor is unavailable (medication certified required)
  • Attend Individuals support plan and IP program meetings as needed or requested.
  • Provide administrative task support to the Program Manager and Instructors.
  • Provide class instruction and presentation as needed.
  • Communicate building maintenance issues and assist in coordination of resolution.
  • Identify Individuals with behavior plans in place.
  • Train communicate and monitor behavior plan protocols with the instructors.
  • Establish and maintain a supportive and professional relationship with individuals their families and circle of support.
  • Provide individual-specific service in compliance with service descriptions as defined in service directory specific attachments and core assurances.
  • Create and maintain an atmosphere of warmth personal interest and positivity throughout the program.
  • Recognize and note physical and/or emotional changes in an individuals condition and seek appropriate intervention. Report all changes to program manager or director within 1 business day.
  • Assist individuals with meals personal hygiene transitioning or other tasks when needed.
  • Assist with daily arrival/ dismissal duties and tasks.
  • Maintain supply inventory communicate needs to the program manager to ensure operation of Individual IP goals and other program classes.
  • Ensure documentation and submission of all incident reports in accordance with the APD guidelines.
  • Communicate all incidents to the program manager.
  • Coordinate community outings ensuring proper coverage and safety protocols.
  • Complete required documentation for services per ADP and Qlarant requirements.
  • Inspect and monitor the building for cleanliness and sanitization per CRN safety standards.
  1. Administration
  • Complete all assigned documentation requirements. Including daily notes and summaries on progress towards support plan/ implementation plan goals.
  • Train assist coach and support instructors on required documentation and content requirements.
  • Contact the program manager to report any difficulties accidents incidents or concerns immediately.
  • Prepare General Event Reports using CRN documentation system and submit to required parties based on APD guidelines.
  • Enter complete notes daily into CRN selected system.
  1. Other Duties
  • Aid and assist individuals onboarding and offloading from transportation.
  • Serve as a positive role model for adults in the program through appropriate dress speech attitude and courtesy.
  • Plan and implement activities that align with each Individuals program goals enrichment and physical activity utilizing curriculum and materials provided by the program.
  • Comply with company policies and procedures.
  • Maintain confidential information and ensure compliance with HIPAA.
  • Attend all trainings as directed and provided by the employer as required of Med-Waiver providers.
  • Other projects and duties as assigned.

SUPERVISORY RESPONSIBILITY

  • Oversight of instructors and life skills program staff in the absence of the program manager.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

  • Primary activities are conducted within a well-lit climate-controlled facility.
  • Occasional community outings may require outdoor travel and potential exposure to unpleasant weather.
  • Ability to bend stand walk or sit for extended periods of time and possess the normal range of body motion.
  • Ability to regularly lift up to 50 pounds independently.
  • Must be able to provide personal hygiene assistance to individuals requiring assistance.
  • Periodic exposure to bodily fluids and other pathogens.

QUALIFICATIONS

  • High School diploma or equivalent.
  • One year of experience in a medical psychiatric nursing or child/adult care setting or direct care with individuals with intellectual and/or developmental disabilities.
  • Valid state drivers license.
  • Must pass Level II Backgroundscreening.
  • Must pass drug screening.
