The Medical Social Worker I (MSW) provides psychosocial support and care coordination from admission through discharge. This role works closely with patients families the Interdisciplinary Care Team and external providers to ensure safe timely and cost-effective care. The MSW develops and monitors care plans advocates for patients and families facilitates discharge planning and serves as a resource for problem-solving and continuous improvement.
Shift: Day Shift PRN/Casual
Key Responsibilities
Assess patient needs across physical cognitive psychosocial and developmental stages; provide age-appropriate care for infants through geriatrics.
Develop implement and monitor individualized plans of care in collaboration with patients families and care teams.
Coordinate discharge planning including referrals to specialty services community resources and third-party payers.
Advocate for patients and families ensuring understanding of insurance and care processes.
Act as liaison with external providers participate in care conferences and ensure timely completion of discharge-related functions.
Engage in outreach education and committee participation to support hospital and departmental goals.
Maintain communication with patients families supervisors and co-workers to support safe efficient patient-centered care.

EDUCATION & EXPERIENCE:

Salary Range: Minimum 1.00/hour - Maximum 1000.00/ will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
