The Community Outreach Coordinator works with DAS and other community partners to strategically increase awareness of ADRC and other relevant IOA programs amongst target populations. They do this by providing non-clinical supervision consultation orientation training and support to all ADRC staff including Site Supervisors and Information & Assistance Specialists and actively engaging and participating in San Francisco community groups and events acting as a steward of IOA to promote services.
Please note: This is a Non-Exempt Temporary role for an employee out on LOA. This role is expected to last 4 months.
Job Description Details:
- Develops best practices for ADRC staff to provide efficient information intake and referral process.
- Ensures that ADRC staff is trained and knowledgeable of all IOA program criteria and other community services available in San Francisco to best meet the needs of the community.
- Meets with ADRC staff via regularly scheduled meetings either in person phone call or virtually serve as point of contact for questions and support regarding consumer follow-ups for referrals and services.
- Ensures ADRC staff are consistent with department and site policies and procedures to meet deliverables and report once a month and as requested.
- Reviews ADRC staff consumer follow-up activity to identify performance support needs and provide consultation coaching and training as necessary.
- Attends relevant trainings and meetings provided by DAS ADRC and IOA geared to improve outreach promotion and sales strategies.
- Maintains monthly ADRC Outreach Calendar and coordinates outreach efforts for individual ADRC sites.
- Maintains consistent communication with ADRC Site Supervisors through at-least quarterly meetings.
- Attends and participates in the creation of the agenda for the quarterly DAS & ADRC Partners meetings.
- Maintains provides and submit Monthly ADRC Carbon Reports to each ADRC Site supervisor.
- Other duties as assigned.
Qualifications:
- Bachelors Degree in gerontology social work psychology or related field or relevant work experience
- Expert knowledge of aging and disability resources in SF/Bay Area
- At least 4 years of case management experience
- Experience and success in working with cross functional teams
- Ability to develop and follow processes to ensure deliverables
- Demonstrated ability to establish excellent internal and external relationships
- Excellent oral/written communication and interpersonal skills
- Computer literacy in MS Word Excel and Outlook required; Procura and RTZ Get Care preferred
COMPENSATION:
- Range: $ 35 - 40/hrly
