Job Summary

PERFORMANCE RESPONSIBILITIES: Coordinates and provides services to students who are returning to school from extended absences related to mental health and medically-based difficulties. Implements and monitors students who are identified as being at-risk providing a preventative coaching model tiered system of support. Assists the STAY team in developing re-entry and prevention plans. Assists in direct service delivery and data collection to inform decisions about management of student mental health performance and workload. Provides case management gathers and maintains data as it relates to goals developed within a students prevention and/or re-entry plan. Tutors and assist students individually in small groups and within the classroom setting to help manage assignments and reinforce concepts presented by teachers. Page 1 of 2 9/12/25 Observes student performance and records relevant data to assess progress. Assists students in managing day-to-day social/emotional and mental health-based issues. Willingness to host supervised lunch and learning opportunities. Interfaces with teachers and related service providers to coordinate instructional efforts reintegration schedule and workload. Works directly with students to develop daily re-entry schedules and support their reintegration back to academic and vocational programming. Provides weekly progress reports to students families teachers and related service providers. Schedules meetings necessary to adhere to the STAY program plan. Provides students within the program the necessary support via assistive devices supportive technology and provides assistance accessing the facilities. Monitors student achievement and/or needs and communicates identified concerns and/or observations regarding student needs to the classroom teacher the Director of Student Support Services clinical counselor and/or the nurse leader. Persists at all times in a professional pleasant manner of respect for the student with mental health-based difficulties and in working in the domain of the teacher/instructor of record. Enforces administration policies and rules governing students. Maintains confidentiality of information concerning students and parents. Performs other tasks or duties as assigned by the Director of Student Support Services. WORK ENVIRONMENT: This position may require long periods of standing and sitting. The individual may be exposed to ultraviolet lights as are found in computer monitors and copying equipment. The individual may be exposed to various hazards in the building. TERMS OF EMPLOYMENT: The work year is 183 days. This includes the two days before the opening day of school Tour Day and the 180 school days. Compensation and fringe benefits will be in accordance with the Paraprofessional collective bargaining agreement in effect. EVALUATION: Annually.
