Job Summary

Job Title: Licensed Practical Nurse (LPN)

Job Type: Contract / PRN
Location: Gloucester MA 1930
Shift Options: 7a 3p 3p 11p 11p 7a
Start Date: ASAP
Contract Length: 4 8 weeks (with potential extensions)
Schedule: 5 days/week with possible weekend rotation

Responsibilities:

Provide direct patient care based on individualized care plans
Administer medications and perform treatments as prescribed
Monitor patient conditions and report concerns to supervising RN or provider
Assist with admissions transfers and discharges
Document care accurately in EMR systems
Collaborate with nursing and interdisciplinary team to support resident health goals
Maintain a safe and clean working environment

Requirements:

Valid LPN license in the state of Massachusetts
Current CPR/BLS Certification
At least 1 year of recent experience in a LTC/SNF
Strong attention to detail and clinical accuracy
Able to work all assigned shifts including weekends if needed
Excellent communication and interpersonal skills
