Job Summary
Social Service Designee is to identify and provide medically-related social services which assist residents in maintaining or improving their ability to manage their everyday physical mental and psychosocial needs. The Social Service Designee reports and is directly responsible to the Director of Nursing however he/she works in concert with the Social Service Consultant.
Responsibilities
Maintains initial interview and collect data and information upon admission quarterly annually change of condition as scheduled and initiate Care Plan as appropriate.
Follows up on daily problems issues complaints lost items related to resident needs and services.
Maintain and make appointments. Referrals to the Dentist Podiatrist Psychiatrist/Psychologist Ophthalmologist Physician office visits.
Consult with Social Service Consultant with any critical/crisis issues and or as needed.
Attends on the spot training with Social Service Consultant which includes care planning assessments proper documentation.
Collaborates with other department and agency related to the needs of resident such as DMV medical offices business offices.
Coordinates and schedules resident needed appointments and arranges and coordinates transportation with the family members public transportation private transportation etc.
Assists the DON any issues that arise on a daily basis.
Assist with investigation process on any issues of suspected abuse and notify family
If room change is necessary informs family member responsible party or resident of impending move fills out and distributes room change notices with proper chart documentation.
Assist with the safe discharge of residents involving family responsible parties and residents.
Ensures confidentiality of residents Social Service information by allowing access only to authorized persons.
Attends in-service and staff meetings as required.
Demonstrates courteous considerate and cooperative behavior when communicating with all facility personnel residents and staff.
Follow up any resident family visitors concern regarding care.
Performs other duties as assigned.
Required Skills/Abilities
Ability to read speak understand English and follow directions and complete the assigned responsibilities of the department.
Support of Social Service Department the facilitys philosophy of care and the policies and procedures of the facility.
Ability to effectively communicate with residents families responsible parties facility staff and outside resources and agencies.
Education & Experience
High school graduate or equivalent.
Job Type: Full-time Day Shift
Pay: $26.00 to $28.00 hourly
