Job Description:
Liquid Personnel is seeking an experienced Occupational Therapist in Wokingham on behalf of its client to assess review and support the housing adaptation needs of vulnerable adults and children. This role aligns with the Housing Grants Construction and Regeneration Act 1996 and aims to promote independence safety and wellbeing through a strengths-based and personalised approach.
Key Responsibilities:
- Conductfunctional assessmentsto determine eligibility and recommend appropriate housing adaptations
- Provideadvice guidance and access to support servicesbased on assessed needs
- Promoteaccessible environments with working knowledge ofbuilding regulationsDisabled Facilities Grant (DFG)legislation and local housing policies
- Assess and orderbasic equipment/adaptationsfollowing major works and provide training on their use
- Undertakemanual handling risk assessments develop risk management plans and support positive risk enablement
- Collaborate with individuals carers families and stakeholders to gather information and identify best-fit solutions
- Participate inDFG panel meetings presenting detailed reports and recommendations
- Monitor and review outcomes to ensurequality assuranceandcustomer satisfaction
- Promote the use ofassistive technology including Telecare to maximise independence
- Maintain accurate and timelyelectronic case recordsin line with GDPR and council procedures
- Share good practice and providespecialist housing OT knowledgeto colleagues and customers
- Support policy development and ensure referral routes follow best practice
- Uphold professional standards and maintainHCPC registration and continuous professional development
Qualifications & Experience:
- Degree/Diploma in Occupational Therapy
- HCPC registration
- Experience inhousing adaptationsDFG assessments and working with vulnerable individuals
- Knowledge ofbuilding regulationsmanual handling andassistive technology
- Strong communication organisational and problem-solving skills
- Ability to work independently and collaboratively within a multidisciplinary team
