An exciting opportunity has arisen to join the dynamic and innovative Young People and Families Service - Rise Up Sefton as an Early Intervention Coordinator.

Rise Up Sefton are a specialist service supporting anyone under 25 who is concerned about their own or somebody elses drug or alcohol use.As a Young Persons Early Intervention Coordinator youll play a key role within a multi-disciplinary team delivering tailored interventions that put young people at the centre. Youll empower them to reduce risk build resilience and lead safe healthy and fulfilling lives.

Full Time Hours: 37.5 per week

Full Time Salary Range: 27861.26 - 32002.35 (based on full time hours pro rata for part time)

*please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Lead on delivering high-quality evidence-based outreach and early intervention across Sefton.
  • Work flexibly to meet the needs of young people and young adults including evenings and occasional weekends using creative and inclusive approaches to engage individuals who may benefit from early support.
  • Coordinate and deliver universal prevention and education offers lead detached outreach in areas of concern and develop peer-led support groups.
  • Collaborate with partners on borough-wide events and proactively generate referrals using local data.
  • Alongside holding a small caseload youll advocate for young people and young adults promote harm reduction and contribute to safeguarding processes.
  • Be part of a dynamic team contributing to service development and ensuring accurate data recording and reporting.
  • Work flexibly and holistically to meet the needs of young people young adults and the service (includes working outside of core hours: evening and occasional weekends)
  • Lead detached outreach in areas of concern (e.g. parks beaches town centres).
  • Deliver high-quality evidence-based outreach activities throughout the Borough of Sefton.
  • Develop innovative engagement methods for hard-to-reach or marginalised groups.
  • Develop & deliver peer led support groups for young people & young adults affected by substances.
  • Collaborate with the Community Education Lead on projects borough-wide workshops and community events (e.g. festivals freshers fairs Pride).
  • Proactively generate referrals using data to target specific services and provisions.
  • Commit to safeguarding young people & young adults adhering to local policies and facilitating referrals where needed.
  • Participate meaningfully in team meetings service development days and supervision.
  • Advocate for and empower young people & young adults promoting harm reduction principles.
  • Be responsible for a small caseload of young people & young adults and to adhere to all aspects of case management policy.
  • To work in a safe & risk informed way ensuring personal safety as well as the safety of colleagues and young people/young adults is at the forefront of outreach delivery.
  • Ensure accurate and timely data recording collection and reporting.

About you:

  • Excellent engagement skills a strong understanding of safeguarding and a commitment to reducing harm and promoting resilience in young people.
  • Experience of working with young people in 121 and/or group
    settings.
  • Experience of working in an outcomes-focussed environment.
  • Ability to forge strong working relationships with internal and external
    partners.
  • Experience of and ability to achieve against key performance
    indicators (KPIs) relevant to the role.
  • Ability to liaise and work in partnership with a wide range of
    professionals and agencies including Childrens Services schools
    primary care
  • The ability to accurately update and maintain records in a timely
    fashion and to work to deadlines for the submission of information
    e.g. reports
  • Ability to communicate confidently and effectively verbally and in
    writing
