About the Role:

We are currently seeking an experienced NDIS Service Coordinator to join our team on a temporary contract basis (38 hours per week) to provide leave coverage.

In this pivotal role you will manage a caseload of up to four Supported Independent Living accommodations supporting teams to deliver high-quality person-centred supports. You will ensure participants are empowered to achieve their NDIS goals exercise their human rights and work towards personal aspirations.

Key Responsibilities:

  • Lead and mentor teams to develop professional practice.
  • Collaborate with other NDIS Service Coordinators to support participants in achieving their goals.
  • Manage a complex caseload ensuring support schedules align with the NDIS Price Guide budget constraints and NDIS Service Agreements.
  • Prepare and submit NDIS Progress Reports in line with plan reviews and reporting requirements.
  • Provide day-to-day leadership supervision and mentoring to staff to ensure high-quality service delivery.
  • Maintain accurate client documentation including risk assessments and support plans.
  • Build and maintain strong relationships with key stakeholders including families allied health professionals and support coordinators.

About You:

You are a dynamic professional with a passion for empowering individuals and demonstrated experience in NDIS case management and/or support coordination. You are confident navigating the complexities of the NDIS framework and bring strong leadership and organisational skills to the role.

Key Selection Criteria:

  • Demonstrated experience in NDIS case management and/or support coordination.
  • Strong leadership skills with experience supervising staff in disability or community services.
  • In-depth knowledge of NDIS standards Human Rights Person-Centred Active Support and restrictive practices.
  • Proven ability to build and maintain strong stakeholder relationships.
  • Excellent organisational and time management skills with the ability to manage competing priorities.
  • Strong administrative and technology skills (client management rostering incident reporting).
  • Relevant qualifications in Community Services Disability or a related field.
  • Current Working with Children Check NDIS Worker Screening and Queensland Drivers Licence.
