The Clinical Program Coordinator is responsible for ensuring the delivery of a high-quality breast and cervical cancer program to address the healthcare and health education needs of clients and the community in an effort to increase screening and establish surveillance of breast and cervical cancer using outreach initiatives educational seminar and individual education opportunities collaboration with partnered and new community stakeholders. Additionally the Coordinator will
ESSENTIAL DUTIES AND RESPONSIBILTIES include the following.
1. Coordinate patient scheduling for mobile mammogram services and liaise with service providers to ensure seamless service delivery
2. Oversee mobile mammogram days and provide support to mobile unit and staff
3. Collaboratively implement outreach protocols that build healthy individuals using CDC evidence-based interventions to increase clinic-level breast and cervical cancer screening rates
4. Identify refer and navigate eligible women to breast and cervical cancer screening and diagnostic services through referrals and community partners
5. Facilitate referral process for diagnostic mammograms ultrasounds not limited to biopsies or any follow-up treatment as ordered
6. Contact patients with abnormal mammograms for follow-up services and provide continuum of care
7. Trains and supervise the Community Health Worker to effectively disseminate health information and manage outreach timelines and projects.
8. Working with the Community Health Worker connect clients with breast and cervical cancer to appropriate medical providers
9. Track patient demographics mammogram services referrals and outcomes on a weekly basis using Microsoft Excel and Electronic Medical Records to maintain an active surveillance
10. Assess individual patient barriers to cancer screening diagnostic services and initiation of cancer treatment and develop access to overcome barriers as allowed
11. Provide patient education and support regarding screening process at risk population risk factors and
12. Assist with creating partnerships to strengthen and enhance program services to program participants including developing and leveraging resources
13. Assist with assessment of program objectives and determine best approaches for achieving program objectives in accordance with client/community needs funder requirements and agency resources
14. Collaborate with and keep all program staff informed of program activities
15. Promotes the departments services via social media platforms giving presentations in the community and developing promotional newsletters/flyers/articles and updating the divisional portion of the Health Districts website via the webmaster.
16. Responsible for disbursing customer satisfaction surveys and compiling a report regarding the findings.
17. Monitors C4 contracts for compliance.
18. Complies with policies and procedures delineated in the current Employee Handbook and Personnel Policies of Brazos County Texas and department supplements
19. Perform other duties within the scope of coordination
SUPERVISORY RESPONSIBILITIES
This is a supervisory position.
QUALIFICATIONS
1. Minimum of one year work experience in a service (individual group or family) environment in womens health or other related field
2. Program design and database management experience recommended
3. Knowledge of relevant best promising and evidence-based practices
4. Strong interpersonal skills and high level of emotional intelligence
5. Excellent verbal and written communication skills
EDUCATION and/or EXPERIENCE
Graduated from an accredited four-year college/university and at least one year experience in a clinical or administrative role; or if no college degree at least two years experience in an administrative or clinical role.
KNOWLEDGE OF
Standard office practices and procedures computers outreach active surveillance practical epidemiological skills division county functions principles and practices.
SKILL/ABILITY TO
Operate a computer operate standard office equipment such as copy and facsimile machines communicate effectively both orally and in writing; maintain effective working relationships with county employees co-workers representatives of other government agencies and the general public and read and write contracts grants and other miscellaneous reports letters and memos.
CERTIFICATES LICENSES REGISTRATIONS
Valid Texas Drivers License and CHW license/certificate within 6 months of employment
