About the Role
The Care coordinator BHHP will provide person centered care coordination for clients living with complex psycho-social chronic healthcare needs while part of BHHP. The Care coordinator will provide clinical intervention as part of a Monday to Friday roster excluding Public Holidays.
Classifications for this position will be Grade 4A Community Health Nurse or Allied Health Grade 2.

About Sumner House
The Sumner House site in Fitzroy has been used to support homeless people through the Better Heath and Housing Program. All services at the site are delivered through a partnership between Brotherhood of St Laurence Launch Housing and St Vincents along with relevant government departments.
The Better Health and Housing Program (BHHP) will support individuals with health issues who are experiencing chronic homelessness by providing an integrated health and homelessness service response for stays of usually 12-26 weeks. BHHP environment will support residents to rest and recover and address health and social issues according to the persons own goals.

POSITION PURPOSE
The Care coordinator BHHP will provide person centred care coordination for clients living with complexpsycho-social chronic healthcare needs while part of BHHP. The Care coordinator will provide clinical intervention as part of a Monday to Friday roster excluding Public Holidays.
POSITION DUTIES
Under the supervision of the Team Leader the Care coordinator will:
Undertake holistic comprehensive assessments in collaboration with the client using a strengthsbased and trauma informed approach.
Collaborative care planning with clients carers and/or families in the context of their healthfunctional psychosocial cultural environmental and economic needs.
Facilitation of client generated goals that are realistic time limited and based on addressinghealth and wellbeing needs.
Identify and access multidisciplinary and specialist supports to address specific client issues.
Assessment and management of risk issues including timely documentation of the plan andescalation.
Monitor health outcomes and plan for the transition and exit from the program ensuringappropriate long term community supports are in place on discharge.
Communicate effectively and professionally with a wide range of team members keystakeholders and external service providers.
Assist/support and at times escort clients to medical and specialist appointments and enabling client appointments.
Work in collaboration with the clients support workers
Administer medications as per medical orders (RNs only)
Communicate with clients in a respectful manner and engage in verbal de-escalation whererequired
Complete all Mandatory Training (Applied Suicide Intervention Skill Training Basic Life SupportOutcome Measurement Infection Control Workplace Culture and Equity Training andManagement of Aggression Training consistent with St. Vincents policy.
Escalate concerns regarding mental or physiological deterioration according to escalationprocedures
Ensure all documentation and client records are completed and managed in a timely manneraccording to SVHM and BHHP and procedures
Actively participate in supervision professional development and performance review
Complete annual clinical competencies.
Act as a resource for staff within an identified area of expertise.
Demonstrate leadership in quality initiatives team processes and service development activities.
INCUMBENT OBLIGATIONS

Complete annual clinical competencies
Maintain skills and knowledge necessary to safely and skillfully undertake clinical work

Participate in clinical risk management and continuous quality improvement activities as part ofday-to-day work
Ensure consumers receive information in an appropriate and accessible format
Actively support consumers to make informed decisions about their treatment and ongoing care
Ensure consumers are aware of their rights responsibilities and how to provide feedback
Protect the health and safety of self and others complying with all health and safety relatedpolicies procedures and directions
Complete required Fire and Emergency Training annually
Complete required Workplace Culture and Equity Training annually
Attend general hospital orientation within 3 months of commencement
As required comply with fit-testing and PPE requirements

SELECTION CRITERIA
ESSENTIAL REGISTRATION LICENSE OR QUALIFICATION REQUIREMENTS
Current nursing or Allied Health registration with AHPRA
