Description
- Identifies patient mental health problems and prescribes treatment.
- Implements mental health services utilizing therapeutic regimens.
- Provides mental health education to patients and their families.
- Provide health/behavioral health assessment (including differential diagnoses prognosis) treatment recommendations and treatment planning for patients across program types.
- Provide medication management for patients on medication and intermittent reassessment (as needed) for those assessed (but not on medication).
- Provide counseling brief therapy psychoeducation about behavioral health and other care for patients as planned by the treatment team and to support the treatment plan of the client.
- Participate in case reviews and contribute to treatment planning goal setting.
- Provide consultations to non-medical staff on complex cases.
- Consult with primary care provider substance abuse counselors blended case managers and other mental health professionals as needed.
- Maintain professional behavior and relationships with all Friendship House staff and referral resources.
- Attend staff meetings provider meetings and other meeting as required.
- Fill out and maintain patient-related paperwork including federal- and state-mandated forms.
- Participate in assessments and medication management off the main site as needed.
- Provide crisis intervention emotional support resources and referrals as needed.
- Comply with Agency policies and procedures including those regarding conduct confidentiality and record keeping.
- Performs in-person and/or telehealth services to identified populations either at the remote location or in the office as directed by agency requirements and policies.
- Perform duties in a professional manner in response to clients co-workers and situations in which own values are challenged.
- Other duties as assigned.
Qualifications
- Board Certified or Eligible in Nurse Practitioner
- Current licensure in Pennsylvania
- 2 years experience working in mental health setting.
