We are currently hiring aFull-Time Community Service Worker to join our Burlington Community Team atAbleLiving Services!

AbleLiving Servicesis a non-profit organization providing personal support services to adults with disabilities living within their own homes or one of our independent living facilities. Our clients direct and customize the services they receive and we support them to maximize their independence and enhance their quality of life. We provide services across the Golden Horseshoe in the Mississauga Burlington Stoney Creek and Hamilton communities.

POSITION SUMMARY:

Under the direction of the Program Coordinator this position is responsible for helping to create effective links between appropriate health and social resources to minimize emergency room visits and hospital admissions; addressing social isolation and anchoring at risk individuals in the community. This position will adopt a holistic approach in helping clients to engage pro-actively with the community to address social determinants of health and wellness. This position works in partnership with other stakeholders including hospital staff primary care and other community organizations with a multi-disciplinary approach to facilitate clients transitioning from one environment to another to support living well in the community.

* This position requires access to a vehicle and a valid drivers license.

* This position may require occasional coverage in both Hamilton and Burlington areas*

EMPLOYMENT GUIDE:

  • University Degree in Health or Social Sciences field or College Diploma with three (3) years work experience inthe Health and/or Social Sciences field.
  • Professional designation is an asset.
  • Experience working with seniorsand extensive knowledge of the local health care system and community resources.
  • Excellent interpersonal oraland written communication skills.
  • Proven ability to work as a team member and demonstrated ability to makesound judgement decision independently.
  • Experience with quality improvement initiatives related to care deliveryprocesses.
  • Demonstrated initiative and experience working independently with minimal supervision.
  • Proficiencywith Microsoft Office programs internet research and ability to learn and master other computer programs asneeded (i.e CareDove Clinical Connect).
