Physical Therapist Long Term Care/SNF - Travel Position

Job Description:
We are seeking a compassionate and skilled Physical Therapist to join a skilled nursing and long-term care facility in Portland OR on a travel contract. The Physical Therapist will evaluate residents develop individualized treatment plans and provide therapy services aimed at improving mobility independence and overall quality of life. The ideal candidate is patient-focused adaptable and experienced in geriatric rehabilitation.
Key Responsibilities:
- Conduct thorough patient evaluations to assess functional abilities mobility and rehabilitation needs.
- Develop and implement personalized treatment plans in coordination with physicians and care teams.
- Provide interventions including therapeutic exercise gait training balance activities and mobility support.
- Educate residents families and caregivers on therapy goals safety practices and use of assistive equipment.
- Document evaluations progress notes and treatment outcomes in compliance with facility and regulatory standards.
- Collaborate with interdisciplinary staff to ensure coordinated care and optimal patient outcomes.
- Promote patient independence safety and quality of life through evidence-based therapy practices.
Qualifications:
- Doctor of Physical Therapy (DPT) or Masters degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Oregon (or compact license).
- Minimum of 1 year of recent experience in long-term care or skilled nursing facilities.
- Current certification in Basic Life Support.
- COVID-19 vaccine and recent tuberculosis test.
- Strong communication clinical and organizational skills.
- Compassionate patient-focused approach with experience supporting geriatric populations.
