Job Summary

North East Ipswich PCN is looking to welcome a qualified and experienced clinical pharmacist into our busy practices to play a full and active role as part of the practice multi-disciplinary team. Full-time or part-time will be considered.

The main responsibilities of the role include:

  • Clinically assessing and treating patients using expert knowledge of medicines for specific disease areas.
  • Providing specialist expertise in the use of medicines and medicine information (i.e. side-effects dosage) across the practice.
  • Providing pharmaceutical medication review and health education advice to patients at the designated practices by face-to-face telephone or notes-based review as agreed with the practice.
  • Providing leadership on person-centred medicines optimisation and quality improvement.
  • Contributing to public health campaigns including flu vaccinations childhood immunisation and adult immunisation programmes.
  • Processing specialist treatment letters.
  • Supporting practice dispensary.
  • Processing hospital medication discharge letters.
  • Acting as a key contact in the practice for all medication related queries.
  • Supporting improvements in clinical care through practice-based audits and implementing change.

About the Candidate

To be considered for this position you will need the following;
  • Qualified pharmacist with 2 years experience current registration with General Pharmaceutical Council.
  • Primary care experience desirable.
  • Confident in use of electronic records and systems; databases and spreadsheets.
  • NMP or working towards NMP.
  • Diploma in clinical pharmacy or pharmacy practice.
  • Leadership activities in previous work.
  • Evidence of consistent pattern of learning from education training and experience.
  • SystmOne experience desirable.
