Client Health Services provides care for clients enrolled in a multitude of programs throughout the division. HIV Services provides testing care coordination treatment case management and preventative services for clients exposed to or living with HIV.
This unique role combines case management for the Ryan White program and prevention skills for identification and prevention of HIV. The Case Manager position is responsible for providing case management for clients living with HIV. This includes assessing client needs referring to community resources ensuring medical needs are met participating in continuous education and quality improvement.
The Prevention Health Specialist promotes health within Yellowstone County by providing HIV Syphilis and Hep C testing for high-risk clients. Staff provide education on safe sex practices harm reduction supplies and education and coordinate referrals for those looking for treatment programs or medical care. This work is performed with integrity and innovation to ensure a high level of customer service delivery.
A. Case Manager/Prevention Health Services 60%
Maintain up to date patient charts progress notes care plans and other related medical information.
Provide patient guidance through financial assistance and additional RiverStone Health services to ensure clients are receiving a full array of services available to them.
Review and submit client request for financial assistance.
Develop and maintain cooperative working relationships with tribal and other local providers agencies and organizations who care for those living with HIV.
Collaborate with tribal and other local agencies and providers to determine community health needs and the availability of services.
Produce maintain and submit regular data and program reports for grant and other reporting requirements.
Participate in and attend meetings conferences workshops and training sessions to become and remain current on the principles practices and new developments in the program field.
Develop conduct or coordinate health needs assessments and surveys.
Develop operational plans and policies necessary to achieve program objectives and services.
Participate in and provide audit data.
Assist in the development of annual work plans and quality improvement projects.
Respond to communications in a courteous and timely manner.
Perform work with the strict knowledge and understanding of protected sensitive patient health information (HIPAA) and confidentiality.
Interview and gather health information using interpersonal and cultural sensitivity skills.
Respond to questions and comments in a courteous and timely manner.
Participate in emergency preparedness trainings exercise and response activities.
Participate in program quality improvement projects.
Attend meetings conferences workshops and training sessions to become and remain current in the principles practices and new development in assigned work areas.
B. Ryan White Case Manager 35%
Enroll eligible clients into Ryan White to assure 100% access and 0% disparity for all persons with HIV in Montana.
Network with local medical dental substance abuse and mental health providers to inform of and educate about Ryan White program.
Coordinate medical dental substance abuse and mental health services for Ryan White clients with local providers to assure continuity of care is accomplished across the spectrum of care.
Facilitate signing of Ryan White paperwork with all providers of care.
Maintain up to date charts for all Ryan White patients getting progress notes lab reports and consultations from all providers.
Participate in program QI projects.
Provide face to face and/or telephone case management for all Ryan White patients to assess the clients current medical status such as medication compliance nutritional issues dental needs mental health/substance abuse issues and financial program assistance.
Enter current medical and demographic data for all Ryan White patients into eCW.
C. HIV Prevention Services
Performs HIV/Hep C testing on Riverstone Healths campus and at a variety of outreach locations
Promotes HIV testing and education and STD education within the Yellowstone Community
Collects and reports data as needed
Provides referrals of new positive HIV to the Ryan White program
Refers clients for PrEP and PEP as need is identified
Participates and coordinates in the Syringe Services Project
Participates in quality improvement and emergency preparedness projects
Develops and presents education as needed to community partners
Perform other duties as assigned in support of Public Health and RiverStone Healths mission and goals.
Minimum Qualifications
High School diploma or equivalent
Any combination of experience and training which provide the equivalent scope of knowledge skills and abilities necessary to perform the work.
Preferred Qualifications:
Bachelors degree in health-related field
Experience working with underserved population
Experience working in a customer service environment
Valid Montana drivers license
Appropriate vehicle insurance and a reliable vehicle to use for occasional day travel
On-the-job training emergency preparedness training in ICS 100 200 and 700.
