Schedule Details:

• 1 full-time position (40 hours / week)
• Will be based at our Eau Claire West location serving multiple Dove Healthcare locations

Join our growing team as an admissions coordinator!

Working as an admissions coordinator you will be responsible for:

• Coordinating admissions with hospital discharge planners and other referral sources
• Assessing coordinating and ensuring a smooth admission transition
• Confirms medical and financial information needed to ensure a smooth admission transition
• Develops and maintains excellent networking skills and relationship building with referral sources
• Ensuring patient continuity of care

Required Qualifications

• Ability to multitask juggle multiple priorities and work in a fast-paced environment
• Proficiency in relevant software tools / applications such as Microsoft.
• Experience with EPIC/Care Anywhere Point Click Care (CRM) is helpful.
• Detailed oriented with strong organizational and time management skills
• Health care experience is desired
• Strong problem-solving abilities
• Proficiency in written and verbal communications
