Youth Case Manager (SNUG)

Title: SNUG Youth Case Manager

Salary: $30.21 per hour

Status: Non-Exempt Full-Time 35 hours per week

Supervisor: SNUG Social Worker

Schedule: Monday Friday 9am 5pm. Regular travel and time spent in school environments is a required part of this schedule.

Function:

The SNUG Youth Case Manager will assist the social work department in providing case management services to victims of crimes with a focus on Middle/High School aged youth in the City of Poughkeepsie. This is inclusive of but not limited to: providing direct service to clients such as navigating systems with which clients may interact meeting basic needs working towards long-term goals applying for compensation through the Office of Victim Services (OVS) and connecting individuals to educational/supportive services. The Youth Case Manager will support youth development healthy relationships and instill positive behavior change amongst vulnerable individuals and groups. The Youth Case Manager will reduce violence in the community support youth carry a client caseload and provide conflict mediation. These activities will be accomplished by using a public health approach to reduce and prevent violence within this population. The Youth Case Manager is central to the success of this initiative and is instrumental in developing monitoring and assessing risk reduction plans for vulnerable youth.

The Youth Case Manager will also be responsible for implementing workshops using evidence-based curricula related to gang and gun violence prevention and use evidence-based curricula to implement weekly support groups for youth

Position Requirements:

• Associates degree from an accredited university
• Experience working in the social services field
• 2 years of experience working in or near the SNUG target area(s)
• Must be able to pass all background checks for work with youth.
• Must be connected to the City of Poughkeepsie Community.
• Ability to work irregular/flexible hours.

The successful candidate will have the ability to work independently and in a collaborative team environment. Excellent communication skills both verbal and written required. Job Responsibilities:

• Work as an onsite case manager and member of the SNUG team in order to support victims of crime affected by community violence with a focus on youth
• Develop and maintain excellent relationship with Poughkeepsie City School District and other community stakeholders
• Conduct outreach to the community to build strong relationships with youth residents and community groups.
• Manage the sites referral resource database and create relationships with local agencies and service providers
• Assist victims of crime to navigate systems including providing court advocacy and transportation to medical appointments
• Work with the SNUG staff and assist them in providing case management services to their high-risk program participants
• Respond with SNUG team to violent incidents in the community attend SNUG outreach events and make home visits to victims and families when appropriate
• Maintain a sufficient caseload of clients at any given time
• Meet with clients as prescribed
• Provide and support Safe Passage for City of Poughkeepsie School District youth.
• Adhere to all documentation and database requirements and accurately track work
• Willingness to travel to trainings and conferences including an initial week-long training that may require overnight travel within NYS and annual two-day conference in Albany
• Be available to support SNUG team in emergency situations regarding incidents with staff or participants
• Ability to work flexible hours (evenings and weekends) when necessary
• Meets with supervisor for weekly supervision
• Communicates effectively and on an ongoing basis with supervisor and SNUG team
• Exercises appropriate judgement when completing job duties
• Maintains records of activities and submits documentation on a timely basis and as requested
• Other duties as assigned

Other Requirements:

• Flexible hours and can travel to different sites attend community meetings community partner offices and out of area trainings as scheduled.
• Complete PYD 101 training administered by Act for Youth
• Complete Health Foundations and Mandated Reporter training administered by OCFS
• Must be reachable outside of office hours if needed.
• Possess valid drivers license.
• Strong computer skills particularly with the Microsoft Office suite.

Work Environment This position is housed in the Family Partnership Center in Poughkeepsie. Regular local travel is required. Candidate must be able to get to and from various work locations throughout the day including Family Services Poughkeepsie City School District locations and community partner offices.
