Title: Registered Nurse Case Manager (RN Case Manager)

We are seeking for an experienced RN Case Manager with a minimum of 2 years of acute medical care management experience focused on care coordination. This is a 13-week travel contract requiring strong clinical background in acute care discharge planning utilization review and regulatory compliance (CMS DOH DRG management HIPAA OSHA Joint Commission). Travel RN experience is required-no first-timers accepted. The RN will work a Monday Friday schedule with weekend rotation and may float to the Laguna facility as needed.
ResponsibilitiesPerform comprehensive care coordination across acute hospital settings.
Manage discharge planning needs assessments and disease management pathways.
Conduct utilization review including appeals and denials management.
Collaborate with interdisciplinary teams to coordinate Plan of Care and ensure safe transitions.
Manage DME orders disability cases and workers compensation cases.
Ensure compliance with CMS DRG DOH HIPAA OSHA Joint Commission and national patient safety goals.
Maintain accurate charting and documentation in Epic.
Provide case management coverage across ED* ICU* Med-Surg and surgical services* patient populations.
Float to the Laguna facility as required.
Maintain a patient ratio of 1:25.
Minimum 2 years of acute medical Case Manager experience specifically performing Care Coordination.
Travel RN experience is required HARDSTOP (no first-time travelers).
Charting system: Epic (Required).
Patient ratio: 1:25.
Weekend rotation: Required.
Floating: Required will float to Laguna facility.
California RN license: Required by start date (pending license accepted at submission).
RTO Restrictions: Maximum of 7 days.
Graduate of an accredited Nursing Program (ASN or BSN acceptable).
2 years acute hospital RN Case Manager experience focused on care coordination (Required).
Experience across emergency ICU* med-surg and surgical services* patient populations (Preferred).
Utilization review and appeals/denials experience strongly preferred.
California RN license (Required; pending accepted but must be active by start).
BLS (Required).
