What Youll Do:
The Care Management Support Specialist works under the supervision of a Tailored Care Manager supporting clients by providing information about and assistance with accessing services and community resources to improve the individuals health outcomes. The Care Management Support Specialist also supports engagement of Tailored Care Management clients and performs in-person activities as needed to adequately support the needs of individuals enrolled in Tailored Care Management.
This position can be based in either Charlotte NC or Greensboro NC.
Education and Experience
- High school diploma or equivalent
- Two years of paid experienceworking directly with childrens mental health
- General outreach andTailored Care Management experience
- Experience with a behavioral health condition or has served as Certified Peer Support Specialist.
- Valid NC or SC Drivers license.
