Job Description
Location: Garner NC
Practice: Rose Hope Primary Care
Job Type: Full-Time On-Site
Schedule: Monday - Friday; 8 AM- 5 PM

What This Role is All About
As a Behavioral Health Care Manager youll be the friendly face helping patients navigate their mental health journey. Youll work closely with medical providers to ensure holistic care.
Responsibilities
Coordinate care with medical providers and mental health professionals
Screen and assess patients for mental health and substance abuse disorders
Provide education and support for treatment options
Track treatment progress and monitor symptoms
Support medication management and adherence
Deliver brief behavioral interventions using evidence-based techniques
Facilitate referrals and consultations
Document patient progress and treatment plans
Requirements
Masters-level licensure in a behavioral health field (e.g. LCMHC LCSW LMFT)
Masters-level associate licensure candidates (e.g. LCMHC-A LCSW-A LMFT-A)
Experience in mental health screening and treatment planning
Strong communication and teamwork skills
Comfortable working with patients in person and by phone
Familiarity with evidence-based interventions and psychopharmacology
Detail-oriented with solid documentation habits
Compassionate and approachable demeanor
