Hospice Certified Nursing Assistant

What Youll Do as a Hospice C.N.A:
Deliver personal compassionate care in alignment with the patients care plan
Support patients and families in learning self-care techniques
Provide comfort dignity and safety during end-of-life care
Communicate important updates to the RN Case Manager and care team
Document patient conditions medication updates and care provided
Take steps to reduce avoidable hospital visits and ensure continuity of care
Serve as a trusted positive presence for patients families and colleagues
What Were Looking For:
High School Diploma or GED
At least 1 year of experience as a Hospice Aide or Nursing Assistant
Completion of a state-approved Hospice Aide training program with competency evaluation (per 42 CFR 484.36)
Home health or hospice experience preferred
Reliable transportation valid drivers license and auto insurance (50% travel required)
Ability to lift 50100 lbs and perform physical caregiving duties
