Responsibilities:
Provide skilled hospice nursing social work and home health aide services
Participate in interdisciplinary team meetings and maintain 24/7 availability for patient care needs
Offer emotional social and spiritual support to patients and families
Ensure compliance with all state and federal hospice regulations
Coordinate with pastoral care and other supportive services
Qualifications:
Licensed hospice care professional (RN LCSW or HHA)
Previous experience in hospice or palliative care strongly preferred
Strong interpersonal communication and documentation skills
