Are you a licensed RN looking for a rewarding opportunity in the Home Health Care industry Cornerstone VNA is seeking a Full-Time Hospice RN to join our compassionate this role youll provide comfort dignity and support to patients and their families during a very meaningful stage of life.

What Youll Do

Provide skilled nursing care to patients in the comfort of their own homes
Coordinate and deliver care that meets each patients unique needs and goals
Adjust care plans in collaboration with physicians and the Hospice Care Team
Support patients and families in fulfilling end-of-life wishes
Ensure comfort dignity and emotional support during each visit
Work independently while knowing support is just a phone call away
Collaborate with our multi-disciplinary team to provide holistic care

What Youll Need

Current RN license in New Hampshire (or a compact state license)
Valid drivers license and vehicle insurance
Strong communication and interpersonal skills
