Job Summary:
This Registered Nurse position requires providing skilled nursing care case management and potentially hospice care to patients of various ages and health conditions. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. A minimum of two years of experience is required.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Providing skilled nursing care including case management and potentially hospice care.
- Maintaining accurate patient records and documentation.
- Adhering to all hospital policies and procedures.

- Ability to work effectively in a fast-paced environment.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work (if applicable).
- Minimum two years of experience in hospital home health or hospice care.

- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- No weekends or holidays.
- No on-call requirements.
- Return-to-office (RTO) must be specified at the time of submission. The facility allows for one call-off per two-week pay period.
- The facility is an acute care hospital and a MAGNET hospital.
