The Care Manager is responsible for coordinating delivery of services to guests in collaboration with the facilitys team members.
Responsibilities
The Care Manager monitors and documents the effectiveness of treatment provided coordinates the admission process and determines with the interdisciplinary team (IDT) appropriate post discharge services.
Coordinates and updates Care Plan. Attends meeting if requested by guest or deemed appropriate by IDT.
Participate in daily IDT meeting provide clinical review as necessary.
Complete assigned sections of Discharge Plan Instructions. Review clinical post discharge instructions with guest upon discharge.
Qualifications
Must possess as a minimum a Nursing Degree from an accredited college or university or be a
graduate of an approved LPN/LVN program.
Must possess a current unencumbered active license to practice as an RN or LPN/LVN through
the State Board of Nursing.
Must have a minimum of two (2) years experience in a hospital nursing care facility or other
related health care facility.
