Job Description

Responsibilities

  1. Identify and screen clients who are accessing services as appropriate to the level of service requested; assess client for eligibility requirements for rent assistance to include TBRA STRMU and PHP.
  2. To maintain full-time employment case manager will provide direct service client contact according to schedule. This includes face-to-face contacts or phone calls with clients consultations about clients with family members or other service agencies up to 4 hours per month in client specific case consultation with the Program Director.
  3. Assist client and caretaker in the implementation of the plan brokeringservices where needed and presenting the case to service provider group for consultation with other involved agencies as appropriate.
  4. Follow-up client evaluate plan effectiveness and revise plan as required.
  5. Provide information and referral to clients as requested or assessed during the course of a session.
  6. Ensure client confidentiality and dignity at all times.
  7. Verify client eligibility (identity seropositive residency income) for program services.



Housing/HOPWA & Financial Assistance

  1. Conduct thorough assessments/screening of clients needs to determine eligibility for HOPWA STRMU and other housing assistance programs
  2. Provide housing information and referrals to community-based housing resources
  3. Help clients complete housing applications understand leases and navigate housing-related paperwork
  4. Develop individual housing service plans and collaboratively establish long-term housing goals with participants aimed at promoting self-sufficiency and stability
  5. Serve as a liaison between clients landlords and other service providers to address disputes and ensure smooth transitions
  6. Ensure all programmatic requirements are met for STRMU assistance including confirming current housing status and verifying need
  7. Implement and adhere to program requirements regarding Housing Quality Standards and lead-based paint regulations including conducting visual assessments if required
  8. Accurately record and document all services provided and maintain comprehensive client files
  9. Coordinate HOPWA services with other case managers and multidisciplinary teams to provide holistic client support
  10. Develop and maintain professional relationships with clients adhering to ethical boundaries
  11. Build and maintain positive relationships with community organizations landlords and other agencies that an support clients in accessing and maintaining housing
  12. Participate in community outreach activities such as health fairs and community events to raise awareness about HOPWA services and engage potential client


Education Training Skills Knowledge and Abilities



  1. High school diploma or GED required Bachelors degree in Social Work Psychology Nursing or Public Health preferred. One (1) year of experience in which an internship or field placement may be considered.
  2. Working knowledge of community resources and special treatment issues for HIV diseases substance use disorder HIV diseases the gay lesbian bisexual and transgender community and community support programs.
  3. Ability to mentally and physically cope with stresses caused by regular exposure to the effects of clients behavioral health issues and respond appropriately to client crises and seek help in coping when needed through supervision case consultation or counseling when needed.
  4. Ability to respond compassionately to persons who are HIV positive and their partners/family.
  5. Ability to develop creative responses to complicated problems involving indigent clients.
  6. A perspective accepting of a wide variety of peoples problems attitudes and lifestyles. Good interpersonal skills and the ability to work on a team.
  7. If in recovery no less than two (2) years substance free.
  8. Access to reliable transportation a current Texas Drivers License and auto insurance
