Internship Description:Complete outreach to adult clients recently discharged from hospital or emergency room to support their transition-home needs (psychiatric and medical) build rapport provide support connect to resources and engage in care. Complete intake sessions with new clients. Complete documentation regarding outreach and service activities. Contribute to creation of outreach materials - letters handout video link etc.
Time Frame: One semester (beginning in September). Interns will work 10h/week and complete 180 hours during their internship.
Skill Development:Resource brokering client engagement and outreach learning about frequently co-morbid medical conditions for individuals with psychiatric conditions active listening and supportive counseling exposure to inpatient and outpatient services
Career Development: This would assist a new BA/BS graduate be more competitive for entry-level positions in social services mental health and community engagement. At KYC they would be eligible for case management and outreach positions
