We are looking for a passionate California Licensed Physical Therapist with a strong interested in working with the frail & elderly population. The perfect candidates has the ability to jump and pick up while working autonomously and on a team. Preventative therapy is essential to this position as is flexibility and the ability to lead and get things done!Evaluates the participants physical condition and functional status establishes a treatment plan with realistic goals and provides appropriate physical therapy interventions.
Instructs and supervises the rehab aide or assistant in provision of maintenance treatment programs.
Provides caregiver training to staff homecare workers and families in the Day Health Center and/or at home as needed.
Participates in the Intake and Assessment Team.
Records the initial and on-going evaluations and makes detailed progress notes using the computerized system. Re-evaluates needs for physical therapy.
Schedules participants for on-going and additional treatment.
Performs home visits hospital visits skilled nursing facility visits as indicated
Provides general instruction on principles of physical therapy to staff members as needed; in-service training for team members.
Provides evening and weekend on-call coverage on a rotational basis.
Meets administrative requirements of the position including participation in meetings.
Actively participates in and encourages actions that promote good public relations with participants their families and friends visitors and the community.
All other reasonably related jobs as assigned.
BACKGROUND AND EXPERIENCE
Interest and demonstrated ability in working with the frail elderly.
Flexibility initiative and ability to work in a interdisciplinary team
Computer literacy preferred.
Language capability preferred but not required Chinese or Spanish
Interest and willingness to work in a multilingual multicultural environment.
Some supervisory experience.
Graduate of an accredited Physical Therapy program.
Valid license as a Physical Therapist in the State of California.
PHYSICAL REQUIREMENTS
Ability to lift up to lift transfer push/pull maneuver and reposition 35 lbs.
Ability to reach bend and walk.
