Job Description

PRIMARY PURPOSE:

The Special Education Transition Specialist support with disabilities in preparing for life after high school including postsecondary education employment and independent living. This role ensures that transition services meet state and federal requirements while collaborating with students families staff employers and community agencies to build meaningful post-school outcomes.



QUALIFICATIONS:

Education/Certification:

  • High school diploma or equivalent required.
  • Associates or Bachelors degree in Special Education Counseling Social Services or related field preferred but not required.
  • Willingness to complete specialized training preferred.

Special Knowledge/Skills:

  • Knowledge of IDEA Texas transition requirements and community resources (training will be provided)
  • Strong communication and organizational skills
  • Ability to build positive relationships with students families and community partners.
  • Flexibility to work across campuses and adapt to diverse student needs

Experience:

  • Experience working with individuals with disabilities in education workforce or community-based programs preferred.
  • Background in vocational training employment readiness or counseling a plus.

MAJOR RESPONSIBILITIES AND DUTIES:

Program Development and Compliance

  • Assist ARD Committees in developing transition-focused IEPs including measurable postsecondary goals and coordinated services
  • Ensure compliance with IDEA and Texas Education Agency (TEA) requirements for secondary transition beginning no later than age 14
  • Support district initiatives related to graduation postsecondary readiness and community-based instruction.

Student & Family Support

  • Help students explore career interests strengths and independent living skills through assessments job readiness activities and community experiences.
  • Provide resources and training to families on guardianship support decision-making and adult service options.
  • Encourage student self-determination and leadership in their IEP planning process.

Collaboration & Training

  • Serve as liaison between the district vocational rehabilitation higher education institutions workforce solutions and community service providers.
  • Partner with teacher counselor and staff to integrate transition skills into classroom instruction.
  • Deliver training and workshops for staff and parents on transition laws resources and best practices.

Data Documentation & Reporting

  • Maintain accurate documentation of transition services as required by TEA
  • Track student outcomes including postsecondary readiness indicators and post-school data.
  • Report progress to administrators and participate in program improvement efforts.

Other

  • Maintain confidentiality.
  • All other duties assigned.
  • Provide personal care as identified in applicable student IEPs.

WORKING CONDITIONS

  • Environment: District-wide travel required to campuses job sites and community agencies
  • Physical Demands: Moderate; includes working in classroom and job setting transporting instructional/job readiness materials
  • Schedule: Follows district calendar with occasional evening or weekend commitments for parents workshops or agency meetings.

  • Maintain emotional control under stress.
  • Regular district wide travel to multiple work locations as assigned
  • Do medium work effort exerting up to 50 pounds of force
  • Occasionally bend and stoop and reach fully above head
  • Possess acute and/or corrected visual and hearing capabilities
  • Possess a high level of mental capability
  • Possess good mental health in order to perform all job requirements
  • Be average or better in energy health and vitality
  • Regular attendance is an essential function.
