The Care Manager RN OR LPN cares for high- risk chronically and acutely ill patients at the direction of a registered nurse and by collaborating with Comprehensive Care team to monitor care and treatment of patients. Responsibilities include regular communication with patients providers and clinical staff to achieve the highest quality of patient care and treatment outcomes. The Care Manager LPN assists in the evaluation of resources and informing development and revisions to the patients care plan. This job will mainly be working in Pickerington Reynoldsburg Gahanna areas Ideal candidate would live in that area and be able to support NE/SE teams.

SECTION III: ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Participate in taking care of high risk chronically or acutely ill patients.
- Provide ongoing feedback to patient primary care team through regular communication in the patient health record or visits into the primary care offices.
- Perform complete assessment of patients current health status including barriers to achieving optimal health and available resources.
- Assist with the editing update and renewal of care plans by providing patient information and clinical data to Care Manager RN.
- Collaborate with care teams and community partners to facilitate continuity of care.
- Manage a case load of acute and chronic care patients under the direction of the Care Manager RN.
- Communicate with patients via phone calls or during scheduled home visits promptly.
- Assist in Transitional Care Management as needed.
- Facilitate referrals to other disciplines both internally and externally to improve patient outcomes.
- Perform other duties as assigned.

- Education Licensures & Certifications
- Required: Licensed Practical Nurse in state of Ohio
- Required: Active Ohio drivers license with the ability to travel throughout COPCs service area.
- Preferred: A minimum of three years prior direct patient care experience in primary care hospice home health or skilled nursing facility setting.
- Preferred: Prior experience with care management in a managed care setting homecare or hospice setting.
- Knowledge Skills & Abilities
- Must be able to perform any clinical or clerical duty as assigned skillfully.
- Ability to administer IM medication
- Ability to perform straight or indwelling catheter procedures blood draw nebulizer treatment etc. as necessary
- Ability to provide same day or next day Home Visits for assessment as needed (ie. lab specimen collection education and treatment plans adjustments with collaboration providers.
- Must demonstrate the ability to handle stressful situations appropriately.
- Must be able to work flexible hours as needed.
- Must demonstrate ability to manage case load and document progress in a timely way as outlined by the care program assigned to each patient.
- Must maintain patient confidentiality.
- Must demonstrate a proficient driving record and up to date auto insurance as travel to visit patients is necessary.
- Knowledge of chronic and acute disease states in adults
- Knowledge of common medications used in a primary care setting including indications dosages and side effects.
- Knowledge of trends in healthcare managed care Medicaid case management medical management and quality improvement
- Proficient in computer software and usage including but not limited to Microsoft Outlook and Microsoft Excel
- Strong analytical organizational and time management skills
- Ability to work independently and within a team environment with minimum supervision.
