Job Description:
The primary purpose of the Basic Physician Trainee (BPT) is to provide effective medical services to patients under supervision while developing expertise in medical practice in a complex clinical environment in inpatient and outpatient areas in the public health organisations within the Network.
Basic Physician Training roles consist of rotations through medical departments working under an appropriate level of supervision by consultants. Rotations have different levels of responsibility and team structures. BPTs will gain increasing clinical autonomy and accountability as team leaders throughout the course of the three-year training program building on their skills and attributes with the goal of entering an Advanced Training program.
BPTs undertaking the care of medical patients should ensure that they operate within their own level of expertise within a collaborative clinical care model and to seek consultation with the treating physician when appropriate in the following activities:
- Clinically assess patients incorporating consultation/history taking examination and formulation of a differential diagnosis and management plans
- Discuss diagnoses and management plans with patients and their families or carers
- Document the progress of patients in multiple settings
- Prescribe medications tailored to patients needs and conditions
- Transfer care of patients
- Choose organise and interpret investigations
- Assess and manage acutely unwell patients
- Plan prepare for perform and provide after care for important procedures
Key Accountabilities
- The BPT will have increasing responsibility and a complex role which will expand clinical expertise whilst undertaking formal training in Adult Internal Medicine through the RACP in a challenging clinical environment.
- Under supervision provide assessment and management of patients presenting with acute medical problems undifferentiated presentations disorders of organ systems defined diseases and common medical presentations across the human lifespan (adolescence to end of life) for various medical specialties in line with rotations to acquire the necessary skills and knowledge to satisfy requirements of RACP specialist physician training and uphold the provision of high quality healthcare.
- Provide high quality patient care throughout the care continuum including admission acute or requested emergency department assessment ongoing reviews management and discharge planning.
- Undertake patient assessment and management arrange for appropriate investigations and consultations and ensure that the approved treatment plan is implemented as well as planning performing and providing after care for procedures relevant to adult medicine.
- Communicate patient-relevant information in a timely and clear manner to patients and their families/carers and everyone involved in the patients care in order to ensure shared decision-making and timely transfer of information. This includes establishing therapeutic relationships with patients/families/carers sharing health care information and treatment plans escalating issues when required respecting confidentiality and privacy open disclosure in the communication of adverse events and notifying the consultant in a timely manner of any significant changes in a patients condition or investigation results that may affect the patients management.
- Prepare and oversee accurate documentation in relation to patients whilst maintaining confidentiality and privacy to ensure timely and efficient communication of patient information and to ensure patient records are effective and accurate for current and future use as needed. This includes preparing treatment plans correspondence with other treating practitioners entry into medical records referrals discharge summaries reports for coroners death certificates and any other documentation in relation to patient care and management.
- Teach and supervise other junior medical staff medical students nurses and allied health staff to ensure an optimal learning environment promote growth and development of others and to enable supervisees to develop the competence and knowledge required for responsible practice.
- Actively monitor and manage own mental and physical wellbeing reporting concerns and seeking assistance if needed.
- Meet RACP training program requirements participate in educational experiences and actively seek to learn and obtain feedbackin order to develop and understand own skills knowledge and capability; and therefore to exercise judgement and know when to seek assistance. This includes participating in the ongoing review and reflection of clinical practice with view to satisfactory progress towards completion of Basic Training; participating in continuing medical education and professional development including but not limited to local Network organised teaching and training; maintaining professional competencies; completing all supervisory report requirements of the RACP; the Network and the LHD/N; actively engaging in performance feedback and the Training Support Pathway if applicable.
Key Challenges
- Maximise the efficient utilisation of the resources of the Health Services while being compliant with the Local Health District/Network and the NSW Health policies and procedures.
- Working in varied clinical settings that include a range of multi-disciplinary teams and supervision systems across metropolitan regional and rural NSW health facilities.
- Balancing and attaining College mandated training and education requirements to develop clinical/procedural expertise and skillset with the delivery of high-level medical care and services in a challenging demanding and varying environment.
Essential Requirements
- MBBS or equivalent current general registration with the Medical Board of Australia.
- A minimum of three (3) years post- graduate experience or two (2) full years of postgraduate experience in public hospital service by the start of the clinical year.
- Current registration as a Basic Physician Trainee with the Royal Australasian College of Physicians (or eligible for) and intent to obtain Fellowship of Royal Australasian College of Physicians.
- This position is subject to an appropriate supervision level which can be supported by the health agency. If the incumbents registration conditions include supervision requirements the level of supervision must be able to be supported by the health agency for this role.
- Valid Working with Children Check.
- Cleared National Police Check.
- To be employed by NSW Health you are required to provide evidence of vaccination to comply with the NSW Health Occupational Assessment Screening & Vaccination against Specified Infectious Diseases Policy this includes any amendments made from time to time.
Other Requirements
- Be prepared to undertake rotations to other hospitals included in the Network as required including non-metropolitan hospitals or to move between services and geographical locations to deliver optimal patient care as directed.
- Compliance with after hours weekend and on-call rosters as appropriately notified.
- Participate in ward rounds and clinical handover to transfer information accountability and responsibility for a patient or group of patients following the ISBAR protocol National Australian Standards. Clinical handover happens within and between Medical Surgical Emergency Paediatric and Critical Care teams.
- Complete mandatory training as required by the hospital local health district or specialty network and NSW Health.
- Attend all applicable organisation-wide hospital department and role orientation sessions.
- Abide by all conditions/provisions of employment as outlined in the relevant Award.
- Conduct all required responsibilities and tasks of the role in a manner that is consistent with legislation local and state-wide delegations procedures policies and manuals which include but are not limited to the following areas:

- work health and safety
- records management
- confidentiality and privacy
- All staff are expected to take reasonable care that their acts and omissions do not adversely affect the health and safety of others that they comply with any reasonable instruction that is given to them and with any policies/procedures relating to health or safety in the workplace as well as notifying any hazards/risks or incidents to their managers.
- Comply with the Royal Australasian College of Physicians training and examination requirements.
- Act as role model and promote a culture and supporting practices that reflect the organisational values through demonstrated behaviours and interactions with patients/clients/employees.
- Comply with supervision requirements as set out by the Australian Health Practitioner Regulation Agency.
- Escalate and document adverse events including bullying and harassment behaviour and doctors in difficulty in a manner consistent with NSW Health policies and LHDN procedures.
- Understand child protection legislation policies and responsibilities associated with notifying and reporting any relevant issues about child protection.
- Complete other duties appropriate to the level and responsibility of the position as designated by the supervising Consultant Network Director or delegate.
- Where possible participate in research projects to contribute to scholarly knowledge that leads to better patient outcomes and to develop skills and understanding of the quality research.
- Participate in the quality assurance activities including RCA/clinical audit activities to actively manage and improve the safety and quality of health care for patients ensure that appropriate standards are maintained gaps in meeting standards are identified and raised and to contribute to the continual improvement of self colleagues the department and the service.
Selection Criteria
- MBBS or equivalent current general registration with the Medical Board of Australia.
- This role requires eligibility for or current registration as a Basic Physician Trainee with the Royal Australasian College of Physicians (RACP) and intent to obtain Fellowship of the RACP. What is your current RACP status

- Proven ability to apply clinical knowledge and concepts in practical situations in the context of multiple competing priorities.
