Meeting Street is currently looking for a Medically Complex Care Consultation Program Coordinator

for 40 hours per week.


Duties:

  • Performs a wide variety of duties including but not limited to the administration and coordination of the MC3 program under the general direction of the Director of Early Intervention.
  • Collects and analyzes program data for MA DPH SDR.
  • Ensures the timeliness and quality of all services provided to children and their families referred to the Medically Complex Care Consultation Program
  • Provides opportunities to the Southeast Region for the following: consultation resources and training and support to families and providers of children with complex medical needs
  • Provides access to sources to obtain equipment including positioning feeding augmentative and assistive technology.
  • Ensures that all policies and procedures are reviewed in a timely manner and are following DPH regulatory and accreditation standards.
  • Collaborates with other programs to ensure a smooth flow of internal and external referrals.

Requirements

  • High School diploma
  • At least two years experience with children with complex medical needs.
  • Valid pediatric CPR and First Aid certification.
  • Understanding of Microsoft Office suite
  • Knowledge of adaptive positioning equipment feeding and augmentative and assistive technology.
  • Extensive knowledge of resources to support children and families with special health care needs.
  • Demonstrates ability to facilitate resolution of specific issues through effective oral and written communication.
  • Ability to interact in a personal and professional manner to ensure that the program functions effectively and efficiently and quality care is provided.
