Job Summary:
This Registered Nurse (RN) position requires a minimum of two years of experience in a hospital setting and involves providing skilled nursing care case management and potentially hospice care to patients. The RN will perform patient assessments administer medications develop treatment plans and collaborate effectively within an interdisciplinary team. Strong communication and collaboration skills are essential.
Location: Petaluma California United States
Responsibilities:
- Patient assessments and monitoring.
- Medication administration and management.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Documentation of patient care and progress.
- Case management (if applicable).
- Hospice care (if applicable).
Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Clinical assessment medication administration and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and automobile for field work (if applicable).
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
Special Considerations:
- On-call responsibilities some weekends.
- Return-to-office (RTO) is required.
- Facility allows one call-off per two-week pay period.
- Acute care hospital environment.
- Eight or ten-hour shifts starting at 0600 five days a week.
Scheduling:
- Primarily eight or ten-hour shifts starting at 0600 five days per week.
- On-call responsibilities on some weekends.
