- Evaluate residents mobility functional limitations and rehabilitation needs through initial and ongoing assessments.
- Develop and implement individualized person-centered treatment plans to improve physical function reduce pain and prevent disability.
- Deliver direct skilled therapy interventions including therapeutic exercises gait and balance training and application of physical modalities.
- Monitor residents progress and adjust care plans as needed to ensure optimal outcomes.
- Collaborate with interdisciplinary care team and participate in care planning including discharge planning.
- Educate residents families and caregivers on home exercise programs and safety to encourage continued independence.
- Supervise and direct Physical Therapist Assistants (PTAs) and therapy aides as per licensure requirements.
