Job Description

Position Summary

This is a full-time position grant-funded by Philly CEAL University of Pennsylvania National Institute of Health and CEO. The Community Health Worker Manager will manage up to 2 CHW Supervisors who supervise up to 8 CHWs in promoting positive health outcomes through care referrals and information sharing while also organizing their local community around the social determinants of health to improve access to health initiatives. This position is part of the Place-Based Initiatives unit and is responsible for identifying the best practices to be applied to the broader CEO. 

Essential Functions

  • Collaborate with Community Health Workers to address and solve issues they experience and support them in completing daily and weekly program goals. 
  • Provide direction and supervision to a team of Community Health Workers daily. 
  • Communicate ongoing project updates to the Community Healthcare Workers team through the Supervisors. 
  • Engaged directly with community residents serve as a bridge between the CEO and the community with a particular focus on communities of color. 
    • Support CHW team in providing one on one coaching and social support to CHW program participants   
    • Provide referrals and improve support for health concerns but not limited to: 
      • ProvideoneononecoachingandsocialsupporttoCHWprogramparticipants. 
      • Provide referrals and improve support forrelated health concerns.
  • Share resources on care for chronic conditions and improve access to city health centers and other federally qualified health centers. 
  • Create and find ways to support the creation in ongoing facilitation of peer-to-peer support systems/networks around the social determinants of health.
  • Other duties as assigned

Competencies Knowledge Skills and Abilities

  • Must be willing to travel to residences homes to conduct home visits. 
  • Musthaveavaliddriverslicenseandaccesstoreliabletransportation. The ability to transport materials routinely moves equipment or supplies weighing up to 30 pounds. 
  • High school diploma or GED required. 
  • Strong candidates will come from the neighborhoods and communities they serve and possess the following: 
    • Interpersonalandrelationshipbuildingskills. 
    • Apassionforservingyourcommunity.  
    • Desiretodevelopandpracticecommunityorganizingskills.

    • Provenabilitytobuildrelationshipsandcollaborate.  
    • Strongoralcommunicationskills.  
    • Strengthintroubleshootinganddevelopingcreativesolutions.  
    • Preferablyhaveexperienceasa communityhealthworkercommunityorganizing or has worked closely with marginalizedcommunities.

Other Specialties Certifications Physical Requirements and Work Conditions

  • Regular predictable attendance is required.  
  • A successful candidate must be a city of Philadelphia resident within six months of hire. 
  • This position will require a combination of offices in remote work. There is an expectation that the Community Health Worker Supervisor will spend extended periods of time doing community/client outreach add public events healthcare settings in client homes. 

Qualifications :

  • Must be willing to travel to residences homes to conduct home visits. 
  • Must have access to a vehicle in a valid drivers license. 
  • The ability to transport materials routinely moves equipment or supplies weighing up to 30 pounds. 
  • High school diploma or GED required.
