Registered Nurse (RN) / Licensed Practical Nurse (LPN) School-Based Individualized Care
Location:
Waterbury Public Schools and associated locations (including in-school transportation and off-campus school-sponsored activities)

Agency Clinical Supervisor (RN) and Waterbury Health Department oversight
Position Summary:
The RN/LPN will provide one-on-one or small group nursing services to students ages 3 22 with complex medical needs in accordance with their Individualized Education Programs (IEPs) Individualized Health Care Plans (IHCPs) and physician orders. This role ensures safe high-quality and continuous care during the school day and when required during transportation and school-related activities.
Key Responsibilities:
1. Direct Student Care
- Deliver individualized nursing care including tracheostomy care suctioning ventilator support tube feedings diabetic care seizure management and other complex medical interventions.
- Administer medications and monitor for adverse reactions per physician orders and school protocols.
- Provide emergency care and implement crisis intervention plans as needed.
2. Compliance & Documentation
- Maintain accurate and timely nursing notes health records and Medicaid documentation in compliance with state and district regulations.
- Follow all City State and Federal guidelines including HIPAA and FERPA confidentiality requirements.
3. Collaboration & Communication
- Work collaboratively with school nurses educators and families to ensure continuity of care.
- Participate in Planning and Placement Team (PPT) meetings and contribute to IEP goal development.
- Communicate with healthcare providers regarding student health status and care plans.
4. Safety & Professional Standards
- Adhere to infection control protocols and universal precautions.
- Ensure compliance with nursing practice standards and ethical guidelines (ANA Code of Ethics).
- Report any changes in student condition promptly to appropriate parties.
Qualifications:
- Education: Graduate of an accredited nursing program.
- Licensure: Current Connecticut RN or LPN license in good standing.
- Certifications:
- Current CPR and AED certification.
- Medication administration certification (or ability to obtain prior to assignment).
- Experience:
- Minimum 1 year of pediatric or school nursing experience preferred.
- Experience with medically fragile children and specialized procedures (e.g. tracheostomy ventilator care).
- Other Requirements:
- Successful completion of criminal background check DCF registry check fingerprinting and drug screening.
- Ability to travel to assigned school sites and accompany students during transportation if required.

Work Schedule:
- Follows the school calendar (approx. 181 days) plus up to 25 days for extended school year services.
- Typical hours align with the student s school day including transportation time (up to 8 hours/day).
