Job Description

The Integrated Health and Outreach Program (IHOP) is a comprehensive service program focused on screening testing treatment and referral for HIV Hepatitis C sexually transmitted infections (STIs) and substance misuse disorders. The Integrated Health and Outreach (IHO) Case Manager position serves as a member of a multi-disciplinary patient care team. They will be responsible for providing case management services that support the treatment goals of each patients care plan. The IHO Case Manager represents the company professionally and positively to enhance and promote the core values and mission of NATIVE HEALTH always exercising the utmost discretion diplomacy and tact interactions with patients staff members external partners and public.

KNOWLEDGE SKILLS AND ABILITIES:

  1. Knowledge of HIV STIs behavioral health case management principles SDOH and community resource coordination.
  2. Knowledge of case management practices.
  3. Two years of related case management experience is required. Experience working with underserved and diverse populations is preferred.
  4. Knowledge of culturally competent care models and trauma-informed approaches.
  5. Positive attitude and the ability to advocate for the patients needs.
  6. Excellent written and verbal communication skills and ability to establish rapport.
  7. Detailed and accurate documentation ability
  8. Ability to navigate multiple service systems and coordinate complex care across agencies.
  9. Proficiency in EHR systems and Microsoft Office; strong organizational and documentation skills.
  10. Professional integrity sound judgment and respect for client confidentiality.
  11. Ability to work collaboratively in interdisciplinary settings while maintaining clear boundaries.
  12. Familiarity with local state and federal behavioral health and social service regulations.
  13. Strong understanding of behavioral health terminology diagnoses and recovery models.
  14. Commitment to continuous learning cultural humility and inclusive practices.
  15. Participate in ongoing training supervision and quality improvement initiatives.

Qualifications :

  1. A bachelors degree in public health social work or other related fields. Masters degree in public health or similar field preferred.
  2. Minimum of two years of experience in HIV and STI prevention or other community health programs preferably within a community health or public health setting.
  3. Bilingual (Spanish/English) preferred not required.
  4. Experience in case management or care coordination in a healthcare or community outreach setting programs preferred.
  5. Experience drawing blood or willingness to become a certified Phlebotomist preferred.
  6. Must obtain Community Health Worker Certification within six months of obtaining role.
  7. Experience in NextGen EHR preferred.
  8. Willing to travel and work flexible hours including evenings and weekends to attend outreach events and meet the needs of the community.

Standard Employment Requirements:

  1. Must possess a valid and unrestricted Arizona drivers license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona drivers license during employment. Must be insurable under NATIVE HEALTHs liability auto policy.
  2. Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  3. Must pass substance abuse testing upon employment and submit to random testing during employment.
  4. Must provide an updated immunization (IZ) record at the time of employment.
  5. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  6. Must obtain BLS certification within (90) calendar days of employment and maintain certification during employment.
