Job Description
Social Worker (Contract Role)
- Location: Cambridge MA 02138
- Duration: 13 weeks
- Shift: Day Shift(8) - 8:00 start
- Guaranteed Hours: 40 hours per week
- Weekly Pay Rate: $2080 - $2240; (Negotiable based on experience)
- Conduct psychosocial assessments of patients and families
- Develop individualized care plans in coordination with medical and support staff
- Provide emotional support crisis intervention and counseling services to patients and families
- Assist patients with discharge planning including referrals to community resources and services
- Advocate for patients needs and connect them to financial housing or legal resources as needed
- Maintain accurate documentation in accordance with HIPAA and hospital standards
- Participate in interdisciplinary meetings and care planning discussions
- Support patients across various departments (e.g. emergency inpatient outpatient) as needed
- Active Massachusetts (MA) Social Work License Required
- Masters Degree in Social Work (MSW) from an accredited program Required
- Minimum of 1 2 years of clinical social work experience
- Strong knowledge of community resources mental health and crisis intervention techniques
