Job Summary:
An opportunity to be part of an Innovative start up program. The first of its kind in King County. This position offers numerous opportunities for professional growth and development. Valley Cities provides outreach to underserved areas of King County. The goal is to assist individuals who may otherwise not have access to behavioral healthcare.
The Mobile Clinician will function as an integral part of a mobile van clinic dedicated to providing recovery oriented person-centered care to clients of underserved regions of King County. Provides individual treatment concentrated support care management duties and community outreach in community settings. Assess clients needs as a part of a screening process for access to care. Assessment of clients needs including housing and other social determinants of health. Address barriers to care coordination for physical and mental health treatment and identify strategies for success.
