Alfred Health is a leader in health care delivery improvement research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne from ambulatory to inpatient and home and community-based services.
The Department
HITH provides acute nursing therapy to patients in the community as a substitution for care otherwise provided in the inpatient setting. Alfred Health HITH cares for a diverse cohort of patients within metropolitan Melbourne.
The Role
As a Clinical Coordinator your role is to assess coordinate and manage HITH patient admissions. You will operate under a care coordination model liaising with in-patient treating units specialist roles members of the multidisciplinary team and community organisations to ensure both patient management and discharge planning is efficiently and effectively implemented.
A key responsibility of your role is the active promotion of the HITH service within the organisation and to external stakeholders. You will support efficiency and innovative ways of working with the EMR within HITH to ensure excellence in patient your role you will demonstrate the core nursing values of Alfred Health which will guide your patient care treatment and management. Your role requires autonomous decision making and demonstrated leadership. You will role model leadership direction and support to the HITH Nurse Clinicians and work with the Nurse Manager and Clinical Nurse Manager to manage a portfolio or quality project.
