Community Resources Specialist General Description
This position is responsible for coordinating a variety of internal and community resources to assist patients. This includes assisting with completion of various applications including but not limited to community resources (transportation housing food and/or medication) and various other programs. This will include working with medical providers community partners local agencies and state organizations.

Community Resources Specialist Essential Duties
- Assist in the development and implementation of organizational care coordination quality improvement initiatives outreach and community resources initiatives.
- Coordinate care by serving as the advocate and resource for the patient and build effective relationships within the community and across the continuum of care.
- Consult with community partners to provide support services and resources.
- Complete daily follow-up calls to patients who visited the Emergency Room.
- Process the Social Determinants of Health screenings.
- Keep comprehensive records of clients progress throughout the process including documentation of phone calls office visits and referrals.
- Work collaboratively with clinical teams to promote continuity of patient care.
- Meets the requirements and competencies to be designated as a community health worker as outlined by the supervisor.
- Maintain working knowledge of local resources.
- Complete all job functions observing confidentiality and HIPAA requirements.
- Attend staff meetings as assigned.
- Perform other duties assigned.
Community Resources Specialist Skills Required
- Ability to establish and maintain effective working relationships with staff patients and the public.
- Knowledge of HIPAA privacy rules.
- Excellent interpersonal verbal and written communication skills with the ability to clearly and concisely communicate with staff patients insurance companies and vendors.
- Ability to work well under pressure manage multiple tasks and prioritize workload.
- Effective and efficient time management including arriving to work prepared and on-time and timely completion of work.
- Computer literacy and ability to read and comprehend written information.
Community Resources Specialist Education and Experience
- High school diploma or equivalent;
- Bilingual Spanish/English with the ability to speak and read both languages fluently is preferred but not required;
- Valid Kansas Drivers license
- Current auto insurance
- BLS certification;
- Social and cultural sensitivity appropriate to ethnically and economically diverse patient and employee base.
