Job Description
This position has designated special entry salary and is non-negotiable.
Pay Grade: D
Under direct supervision provides a broad range of paraprofessional social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes participates in treatment team planning and performs crisis intervention.
Job Responsibilities:- Ensures database information is maintained on case intake and dispositions.
- Assists clients in locating and utilizing community resources including legal medical financial assistance and other referral services.
- Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients overall progress.
- Provides short-term case management and referral services to clients with emergency situations.
- Routinely enter data into automated system in order to add or update member records.
Core Competences:- Strong knowledge and experience of Division of Family and Children Services programs and process.
Minimum Qualifications
High School diploma/GED andone (1) year of job- related experience in a social services-related position.
Preferred Qualifications:
Preference will be given to candidates who in addition to meeting the minimum qualifications demonstrate some or all of the following skills/experience.- Prior experience working in a child welfare related field.
- Experience using DFCS social services system SHINES.
- Reliable transportation and valid drivers license for work-related travel.
